Faith Formation Opportunity
Please consider joining the Faith Formation team this fall. We need YOU to share your faith with OUR children, youth and teens. Call the Faith Formation Office to learn more about this ministry.

Attendance
Classes begin at 6:45. Please call the Faith Formation Office at 266-8711 or reply to this email if your child will not be with us. We want everyone to be safe and we take attendance. We miss the members of our community when they are not with us.

Drop Off & Pick Up
Please use caution and care as you drop off and pick up your child(ren) on Wednesday nights. Thank you!!!

Weather Cancellations
Remember: If Cedar Falls Schools cancel or dismiss early due to the weather, Faith Formation does not meet that evening.

PLEASE...NO FOOD OR DRINK AT RE!
I've noticed that many of our children, youth and teens are bringing food and drinks into the building on Wednesday nights.
I would ask that if you send food or drink items with your kids that you send enough for their entire class. We are trying to reinforce the good manners that are being taught at home!

Forms
The Archdiocese of Dubuque requires that the following forms be on file for all youth and teens who participate in church activities.
Please complete a one-time 2009/10 Annual Parental/Guardian Consent Form and Liability Waiver for your child(ren). This form is good for the entire 2009/10 school year, beginning June 1. It will be kept on file in the RE/YM Office.
A Minor Hold Harmless/Indemnity Agreement and an Off-site/Field Trip Permission Form need to be filled out for each activity.

Office know if you have any questions.
Return forms to the RE or Parish Office to the attention of Amy Hoyer. Thank you.
